CNA – LTC​/SNF

Job Summary:

Assist patients in their daily living activities within a long-term care or skilled nursing facility setting. Provide compassionate, high-quality care while adhering to facility policies and safety protocols.

Responsibilities:

• Provide assistance with activities of daily living such as bathing, dressing, and grooming.
• Monitor and document patient vital signs and changes in condition.
• Support patients with mobility, positioning, and transfers.
• Follow care plans and report any concerns or changes to nursing staff.
• Maintain a clean and safe environment for patients and staff.
• Comply with infection control and safety procedures, including COVID-19 protocols.

Qualifications:

• Valid CNA certification and active license.
• Proven experience working in LTC or SNF environments.
• Ability to demonstrate compassion and excellent communication skills.
• Capable of performing physical tasks including lifting and repositioning patients.
• Reliable, punctual, and able to work flexible shifts.
• Knowledge of infection control and safety regulations.
