General Statement of Duties: The Registered Nurse (RN) provides nursing care to patients and families based on assessed needs reflected in the plan of care.

Level of Responsibility: Reports to Nurse Manager, Cacapon team

Status:Non-exempt, hourly, full-time

Pay Range:$37.25/hour - $46/hour, additional compensation may be available for additional education/certifications

Schedule:Monday-Friday (8 or 10 hour shifts), back up on call rotation

• Conduct comprehensive assessments of patients' physical, psychosocial, and safety needs to develop and update individualized plans of care.
• Coordinate and provide direct nursing care, admissions, symptom management, and patient visits in accordance with the established plan of care.
• Collaborate with the interdisciplinary team, patients, and families to establish, revise, and maintain plans of care that support quality end-of-life care.
• Educate and support patients, families, caregivers, and facility staff regarding hospice philosophy, disease progression, pain management, and symptom control.
• Communicate patient status changes with physicians, obtain and implement orders, and coordinate medication and treatment updates as needed.
• Establish and maintain positive working relationships with patients, families, physicians, community partners, and nursing facility staff to ensure continuity of care.
• Document assessments, interventions, patient/family education, physician communications, and other required clinical information in a timely and accurate manner.
• Assess, supervise, and coordinate hospice aide services, including development and ongoing evaluation of aide care plans.
• Provide after-hours support, phone triage, crisis care, and on-call coverage as assigned, responding to patient and family needs in a timely manner.
• Support care within nursing facilities by participating in care planning meetings, assisting with wound care management, identifying hospice-appropriate patients, complying with facility requirements, and serving as a hospice resource for facility staff.

Additional Responsibilities: Participate in staff meetings and educational programs, assist with inpatient facility staffing as needed, maintain organizational productivity standards, and perform other duties as assigned.

Requirements

Education & Experience:

• Ability to establish and maintain good working relationships with the patient and family, the attending physician, coworkers and outside agencies.
• Current West Virginia eNLC Registered Nurse (RN) license is required. New RN graduates - please share when you're taking the NCLEX exam.
• One year nursing experience, home health or medical surgical nursing in an inpatient setting preferred.
• Provides own transportation, and a license to drive with current liability insurance. Driving record meets Hospice’s insurance company standards.
• Computer proficiency required.

Physical

Qualifications:

• Ability to transport self to office and patient care settings throughout the service area.
• Ability to walk from car to patient’s room in their care setting and perform essential job functions.
• Ability to lift and move patients and equipment. Able to lift 50 pounds.
• Ability to communicate verbally and in writing in English language.
• Ability to hear normal conversation in person and via phone.
• Ability to read normal or condensed print.
• Ability to use a computer
• Ability to withstand odors.

Equipment Used:
Telephone, computer, office equipment, and medical equipment and supplies in order to provide patient care.
