Fairmont Rehabilitation & Healthcare Center is looking for a dedicated Wound Care Nurse to join our team. This is a great opportunity for an experienced RN or LPN who takes pride in providing quality clinical care and wants to play an important role in helping our residents heal, maintain skin integrity, and improve their overall quality of life.
As our Wound Care Nurse, you will work closely with residents, nursing staff, providers, and the interdisciplinary team to assess wounds, develop and monitor treatment plans, and ensure care is consistently provided according to current standards and best practices.
What You'll DoAssess, monitor, and document residents' wounds and skin conditions
Develop and implement individualized wound care treatment plans
Perform wound treatments and ensure treatments are completed appropriately
Monitor wound healing and communicate changes to providers and the interdisciplinary team
Maintain accurate and timely clinical documentation
Identify residents at risk for skin breakdown and assist with prevention strategies
Collaborate with nursing staff to ensure appropriate wound care techniques are followed
Educate nursing staff, residents, and families on wound prevention and treatment
Participate in care planning and quality improvement initiatives
Monitor treatment effectiveness and recommend adjustments as appropriate
Maintain compliance with facility policies, regulatory requirements, and infection prevention standards
Serve as a clinical resource and advocate for residents requiring specialized wound care
Current RN or LPN license
Wound care experience preferred
Long-term care/skilled nursing experience preferred
Strong clinical assessment and documentation skills
Knowledge of wound prevention and treatment practices
Excellent communication and organizational skills
Strong attention to detail
Ability to work independently while collaborating effectively with the nursing and interdisciplinary teams
