Reablement Outreach Practitioner
Location: Wakefield, West Yorkshire
Salary: £20,298.12 – £22,374.22, Grade 6 (approximately £28,598 – £31,537 for full‑time equivalent)
Job Type: Full Time
Contract: Permanent, working 26.25 hours per week.
The Reablement Service operates 7 days a week, 52 weeks a year, supporting people across the district to regain independence and live safely a Practitioner you will lead a dedicated team to deliver person‑centred care to some of the most vulnerable people in our community.
Responsibilities- Lead a dedicated team of carers to provide person‑centred care.
- Work on a 4‑week rota, including weekends and bank holidays, with shifts between 06:30 – 15:00 and 14:30 – 23:00.
- Provide care and support in the person’s own home and for those waiting for a long‑term package of care or needing to bridge a period before care can restart.
- Ensure individuals are treated with dignity and respect and work with other professionals and agencies to deliver effective services within national guidelines and standards.
- Salary: £28,598 – £31,537 (FTE) plus enhancements for weekend and bank holiday working.
