Location: Kearneysville
Home Base Inc. – Statewide (West Virginia) seeks a Wraparound Facilitator/Case Manager full-time to support families within the Behavioral Health Program. You will provide wraparound services to a caseload, coordinate supports, and lead planning from engagement through discharge.
You will collaborate with multidisciplinary partners, conduct CANS assessments, and ensure timely documentation while working evenings and weekends as needed. Bachelor's degree required.
