Home Base Inc. is seeking ambitious, driven, and enthusiastic applicants for the position of Wraparound Facilitator / Case Manager within our Behavioral Health Program. This role is ideal for anyone wanting to support children and families while gaining valuable professional experience in a reputable and established organization.

- Bachelor’s Degree in a human services field
- Valid driver’s license and reliable transportation
- Ability to pass NCIC/CIB and APS/CPS background checks
- Three professional references
- Strong written and verbal communication skills
- Proficiency with Outlook, Word, Excel, and PDF platforms
- Professionalism and strong customer service skills
- Ability to work independently with minimal supervision
- Ability to work evenings and weekends as needed
- Experience with virtual platforms
- 2–4 years of case management and/or behavioral health experience
- 1–2 years of experience leading teams
- Experience providing direct services to children and families
- Provide wraparound services to a caseload of up to 15 families
- Coordinate supports, resources, and services using a strengths-based approach
- Facilitate team meetings with youth, families, and multidisciplinary partners
- Collaborate with MCO care managers and community resources
- Conduct and administer the Child and Adolescent Needs and Strengths (CANS) assessment
- Monitor service implementation and ensure families receive identified supports
- Lead planning from engagement through transition and discharge
- Conduct home visits according to each family’s individualized plan
- Identify service gaps and collaborate with community providers to address needs
- Maintain accurate, timely, and compliant documentation
- Participate in scheduled supervision and coaching
- Maintain client records in alignment with agency standards and regulatory guidelines
- Ability to walk up and down stairs
- Ability to lift 5–50 pounds
- Ability to kneel, bend, squat, and stand
- Ability to operate a motor vehicle and follow all state traffic laws
