What You'll DoAs a Foster Care Coordinator, you will:

Manage a specialized foster care caseload while ensuring child safety, permanency, and well-being.

Build meaningful relationships with foster families through ongoing coaching, guidance, and support.

Conduct monthly home visits and maintain regular contact with children and caregivers.

Develop, implement, and monitor individualized treatment and permanency plans.

Coordinate services with schools, healthcare providers, therapists, courts, and community agencies.

Facilitate multidisciplinary team meetings and family visitation.

Prepare timely documentation, progress reports, court reports, and electronic case records.

Complete annual foster home reevaluations and ongoing foster family support activities.

Participate in court proceedings and provide professional testimony when required.

Respond to crisis situations and participate in an after-hours on-call rotation.

Support accreditation, Continuous Quality Improvement (CQI), and agency strategic initiatives.

What We're Looking For:

Required Qualifications Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a closely related field from an accredited college or university.

Current West Virginia Social Work license.

Valid driver's license with an acceptable driving record.

Ability to travel throughout assigned service areas.

Successful completion of all required background checks.

Preferred Qualifications Experience in foster care, child welfare, behavioral health, or family services.

Knowledge of trauma-informed care, child development, and permanency planning.

Strong assessment, documentation, organizational, and communication skills.

Experience working collaboratively with multidisciplinary teams and community partners.
