Wise Path Recovery is seeking a dedicated Case Manager to join our Williamson, WV team. You will coordinate services, advocate for clients, and connect them with housing, employment, transportation, and community resources to support sustained recovery.
You will work within a multidisciplinary team to create individualized care plans, document progress in the EHR, and ensure smooth transitions of care from treatment to community living.
