At Harmony Hospice, we are committed to connecting patients and families with compassionate, high-quality hospice care. The Hospice Community Liaison develops and maintains strong relationships with healthcare providers and community partners to increase awareness of hospice services and support census growth.
This is a non-clinical position focused on education, relationship building, community outreach, and referral development. The role does not require clinical licensure or involve direct patient care.
Responsibilities for Hospice Community Liaison:Develop and maintain strong referral relationships with hospitals, skilled nursing facilities, assisted living communities, physician offices, and other healthcare providers.
Conduct regular in-person visits with referral sources to build relationships and identify new referral opportunities.
Educate healthcare professionals, patients, families, and community organizations on hospice services, eligibility, and benefits.
Provide community education, presentations, and in-services to promote awareness of hospice care.
Serve as a primary liaison between referral sources and the hospice team.
Communicate referral information accurately to intake and clinical staff and assist with smooth transitions of care.
Collaborate with admissions and intake teams to support timely referrals and admissions.
Document sales and referral activity and work toward established monthly and quarterly growth goals.
Monitor referral patterns, local market trends, and community needs to identify opportunities for growth.
Participate in community events, networking opportunities, and strategic outreach initiatives.
Maintain knowledge of hospice services, referral processes, payer sources, and discharge planning workflows.
Represent the organization professionally and provide excellent customer service to referral partners.
Bachelor’s degree in healthcare, marketing, business, or a related field preferred.
Minimum of two (2) to three (3) years of healthcare marketing, sales, or business development experience preferred.
Hospice or home health experience strongly preferred.
Strong relationship-building, networking, communication, and presentation skills.
Proven ability to develop referral relationships and meet growth goals.
Strong organizational and time management skills with the ability to work independently.
Knowledge of hospice philosophy and services preferred; training may be provided.
Valid driver’s license and reliable transportation required.
This position involves frequent local travel to healthcare facilities, physician offices, and community organizations, with occasional evening or weekend community events. The role includes both office and field-based responsibilities.
