Certified Nursing Assistant; CNA

We are seeking a Full‑Time Hospice Aide (CNA) to join our compassionate team and provide hands‑on care to patients at the end of life.

Continuum Hospice is a mission‑driven, patient‑centered leader in end‑of‑life care. In this role, you will work closely with the nursing team to ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Monday – Friday, 8 am – 4:30 pm with weekend rotation.

• Provide personalized care following the plan of care to ensure comfort and well‑being.
• Assist with personal care tasks such as bathing, oral hygiene, changing bedding, clothing, and shampooing.
• Deliver exceptional customer service and support families throughout the care process.
• Provide support for families by sharing knowledge and expertise and guiding them in caring for loved ones.
• Maintain accurate documentation in accordance with agency and regulatory standards.

• High School Diploma or G.E.D.
• Current unencumbered CNA certificate in the state.
• Minimum one year of CNA experience in acute care or long‑term care; hospice experience a plus.
• Current CPR certification if state‑mandated.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Pay: $21.00 – $22.00 per hour (average). Final compensation will be discussed during the interview process.
