United Health Group is hiring a Full-time Home Health RN Patient Care Manager in Massachusetts. You will supervise clinical services, coordinate an interdisciplinary team, and ensure continuity of high‑quality care in line with physician plans and state and federal regulations.
Responsibilities include accepting referrals, assigning clinicians, evaluating orders, and coordinating benefits and approvals. Current MA RN licensure is required; CPR is preferred.
