Summary:

The Clinic-Based Pharmacist
- Infectious Diseases provides advanced clinical pharmacy services for patients with HIV and viral hepatitis and supports the delivery of specialty pharmacy services across Maine Health.

The pharmacist will serve as the primary pharmacy subject-matter expert for virology across Maine Health and will work collaboratively with infectious diseases providers, specialty pharmacy staff, ambulatory and inpatient care teams, and other pharmacy colleagues.

As a member of the five-pharmacist infectious diseases team, the pharmacist will also provide cross-coverage and support for antimicrobial stewardship and other infectious diseases pharmacy services based on patient care and organizational needs. The position shares responsibility for weekend, holiday, and other scheduled coverage requirements within the infectious diseases pharmacy team.

Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education:

    Bachelor of Science in Pharmacy or PharmD degree from a recognized/accredited school or college of pharmacy required.
  • License/

    Certifications:

    Current applicable state(s) license with Board of Pharmacy required.
  • Experience:

    Previous pharmacist experience preferred.
  • Key Responsibilities Virology Clinical Practice
    • Serves as a pharmacy subject-matter expert for the treatment and prevention of HIV and hepatitis across Maine Health.
    • Recommend evidence-based treatment and monitoring plans for patients receiving antiviral therapy in ambulatory, specialty pharmacy, and inpatient settings.
    • Collaborates with physicians, advanced practice providers, nurses, social workers, care managers, pharmacy technicians, and other members of the healthcare team.
    • Participates in the development, review, and implementation of virology-related clinical guidelines, protocols, order sets, policies, medication-use initiatives, and formulary reviews.
    • Provides education and counseling to patients and caregivers regarding antiviral medications, treatment expectations, adherence, adverse effects, drug interactions, and appropriate medication use.
    • Support onboarding and training of pharmacists as well as pharmacy students and residents
    Specialty Pharmacy Services
    • Provides comprehensive medication management for patients receiving specialty medications within assigned areas of practice.
    • Supports patient onboarding, education, adherence assessment, clinical monitoring, follow-up, and therapy reassessment.
    • Collaborates with specialty pharmacy team members to address medication access, insurance requirements, prior authorization needs, financial barriers, and continuity of therapy.
    • Identifies and resolves medication-related problems, including adverse effects, drug interactions, adherence concerns, therapeutic duplication, and gaps in monitoring.
    Preferred Skills/Qualifications
    • Completion of PGY2 residency program in infectious diseases
    • Board Certified Infectious Diseases Pharmacist (BCIDP) and/or HIV Pharmacist credential (AAHIVP)
    • Familiarity with HIV and hepatitis treatment guidelines and therapies
