Optum Home & Community Care, part of United Health Group, provides integrated care for patients in nursing homes and assisted living. As a member of the Senior Community Care product team, you will support patients during transitions from skilled rehab to home settings, ensuring smooth care transitions and preventing readmissions.

• Perform comprehensive assessments and document findings in a concise and compliant manner per CMS regulations.
• Ensure all diagnoses are accurately coded in ICD‑10 and fully documented.
• Address all required quality elements during each visit.
• Conduct an initial medication review.
• Complete mandatory educational and virtual training requirements.
• Travel between care sites as required.
• Care Coordination:
  Communicate medical concerns with the patient and relevant team members during visits.
• Program Enhancement Expected Behaviors:
  • Develop innovative approaches to care improvement.
  • Work independently with minimal supervision.
  • Submit available hours to the web-based scheduling system at least one month in advance.
  • Demonstrate initiative toward individual, team, and organizational goals.
  • Participate in quality initiatives for the SCC program.
  • Check Optum email intermittently for required trainings, communications, and scheduling.

• Certified Nurse Practitioner through a national board.
• Graduate of an accredited master’s nursing program (MSN) with board certification through AANP or ANCC (Adult‑Gerontology Acute Care, Adult/Family or Gerontology). Preferred certifications: ANP, FNP, or GNP.
• Active and unrestricted license in the State of Maine.
• Current DEA licensure or prescriptive authority, or ability to obtain per state regulations.
• Availability to work at least 10 hours per week.
• Driver’s license and reliable transportation to travel to patient sites.

• 1+ years of post‑graduate experience in long‑term care.
• Ability to lift a 30‑pound bag and navigate various dwelling configurations.
• Knowledge of geriatrics, chronic illness, and advanced illness/end‑of‑life discussions.
• Proficient computer skills for documenting in electronic medical records.
• Proven ability to develop and maintain positive customer relationships.

Hourly wage: $43.17 to $84.95 per hour for full‑time employment. Pay is based on local labor markets, education, experience, and certifications.
