LVN ​/ LPN – LTC​/SNF

Long-Term Care Nursing Position

Provide compassionate and skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure residents’ safety, comfort, and well-being through professional clinical practices.

Responsibilities:

• Administer medications and treatments in accordance with physician orders and facility protocols.
• Monitor residents’ health status and document changes or concerns promptly.
• Collaborate with interdisciplinary teams to develop and implement personalized care plans.
• Ensure compliance with state and federal healthcare regulations and facility policies.
• Educate residents and their families about health conditions and care procedures.
• Maintain accurate and detailed documentation of patient care activities.

Qualifications:

• Valid LVN or LPN license in the state.
• Minimum of 1 year experience in LTC/SNF environments preferred.
• Demonstrate strong clinical assessment and documentation skills.
• Ability to work independently and as part of a healthcare team.
• Must have current CPR certification.
• Must be able to bring a valid state or federal-issued  wear a photo badge at all times.
