LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Seeking a qualified Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a contract position in Long-Term Care/SNF settings. The nurse will provide quality patient care in a fast-paced healthcare environment.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and document patient conditions and report any changes to the healthcare team.
• Assist with patient assessments, vital signs, and daily living activities.
• Adhere to all safety and infection control protocols, including wearing identification badges.
• Ensure compliance with vaccination policies and maintain required documentation.

Qualifications:

• Hold a valid and current LVN or LPN license.
• Demonstrate a minimum of 1 year of experience in LTC or SNF settings.
• Provide proof of COVID-19 vaccination or valid exemption documentation.
• Ability to follow clinical policies and procedures accurately.
• Excellent communication and patient care skills.
