RN Clinical Assessor

• Complete medical and/or behavioral health assessments within contract requirements
• Conduct on-site, video call, and/or telephonic assessments to determine individuals' needs for services and supports, eligibility, level of care, or related outcomes
• Evaluate individuals in homes, group homes, and care facilities to determine Long Term Care needs and appropriate placement options
• Perform all duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
• Commute within and cover Cumberland County
• Travel as required by program contract requirements
• Perform other related duties as assigned

• Current Registered Nurse (RN) license valid in the state of practice is required
• High School Degree or equivalent required
• Minimum 1 year of clinical experience required
• Minimum two years of post graduate nursing experience
• Preferred: experience in long-term care or home health care
• Preferred: experience with OASIS or MDS
• Ability to commute within and cover Cumberland County
• Ability to conduct on-site, video call, and telephonic assessments
• Compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
• Reliable high-speed internet service: minimum 20 Mbps download/50 Mbps for shared internet connectivity and minimum 5 Mbps upload speed
• Private and secure workspace

Demonstrates expertise in conducting comprehensive medical and behavioral health assessments while ensuring compliance with HIPAA guidelines and Person First standards. Proficient in evaluating long-term care needs and providing appropriate placement options in various settings.
