LVN​/LPN LTC​/SNF

Job Summary:

Provide high-quality nursing care to residents in long-term care or skilled nursing facilities as an LVN/LPN. Ensure adherence to clinical standards and promote patient well-being through compassionate and efficient care delivery.

Responsibilities:

• Administer medication and treatments in accordance with physician orders and facility protocols.
• Monitor residents’ health status, noting changes and reporting significant findings to the healthcare team.
• Assist with activities of daily living, including bathing, dressing, and feeding residents.
• Document patient care accurately and maintain detailed medical records.
• Collaborate with multidisciplinary team members to develop and update individualized care plans.
• Follow infection control protocols and assist with maintaining a safe, compliant environment.

Qualifications:

• Current LVN or LPN license valid in the state of practice.
• Demonstrate clinical competence in geriatrics or long-term care settings.
• Complete mandatory vaccine requirements or provide valid exemptions.
• Ability to effectively communicate with residents, families, and team members.
• Strong attention to detail and organizational skills.
• Ability to wear identification and photo badges at all times during shifts.
