Job Summary:

The Licensed Vocational Nurse (LVN/LPN) provides essential nursing care to residents in a long-term care or skilled nursing facility setting. The role ensures residents receive safe, effective, and compassionate care in accordance with regulatory standards.

Responsibilities:

• Administer medication and treatments accurately according to care plans.
• Monitor residents’ vital signs and overall health status, reporting changes promptly.
• Assist with activities of daily living, including bathing, dressing, and mobility support.
• Document resident care and observations thoroughly and consistently.
• Collaborate with the healthcare team to develop and update resident care plans.
• Follow infection control protocols and maintain compliance with safety standards.

Qualifications:

• Valid state-issued LVN/LPN license or certification.
• Proven experience working in LTC/SNF or similar long-term care environments.
• Ability to communicate effectively with residents, families, and team members.
• Knowledge of medication administration, safety procedures, and regulatory compliance.
• Flexibility to work scheduled shifts and adapt to facility needs.
• Current CPR and First Aid certification.
