Licensed Practical Nurse

Springbrook Center - ME is seeking a Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehabilitation facility in Skowhegan, ME. The LPN will deliver hands-on clinical care, including medication administration, treatments, and wound care, while closely monitoring residents' conditions and documenting in the EHR. You will collaborate with RNs, therapists, and the interdisciplinary team to support individualized care plans, rehab goals, and quality outcomes.

Responsibilities

• Provide hands-on nursing care to residents in skilled nursing and long-term care units
• Administer medications and treatments per provider orders and facility protocols
• Monitor and document vital signs, resident status, and responses to care in EHR systems
• Support rehabilitation goals by assisting with therapy plans and mobility activities
• Collaborate with RNs, therapists, and care team to update and follow individualized care plans
• Educate residents and families on care, treatments, and health management
• Maintain infection control, safety, and regulatory compliance standards
• Report changes in resident condition promptly to supervising nurse or provider
