LVN​/LPN LTC​/SNF

Job Summary:

Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting as an LVN/LPN. Deliver essential clinical services with an emphasis on patient comfort, safety, and well-being in a team-oriented environment.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and document resident health status and report significant changes to the charge nurse.
• Assist with activities of daily living, including bathing, dressing, and mobility support.
• Follow infection control protocols, including COVID-19 vaccination requirements and other safety guidelines.
• Communicate effectively with residents, families, and healthcare team members.

Qualifications:

• Valid state or federal-issued LVN/LPN license.
• Current certification in CPR and basic life support (BLS).
• Proven experience in LTC/SNF or similar healthcare environments preferred.
• Ability to adhere to facility policies regarding vaccination and safety procedures.
• Strong interpersonal and communication skills.
