LVN​/LPN – LTC​/SNF

Job Summary:

The LVN/LPN will provide quality nursing care to patients in a long-term care or skilled nursing facility. This role involves supporting the healthcare team in delivering efficient and compassionate patient services.

Responsibilities:

• Administer medications and provide basic patient care as directed by the supervising RN or Physician.
• Document patient information accurately and maintain detailed patient records.
• Monitor patients' health status and report any changes or concerns promptly.
• Follow infection control procedures and ensure compliance with safety regulations.
• Assist with patient mobility and activities of daily living.
• Ensure proper use of medical equipment and maintain a clean, safe environment.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1 year of experience in long-term care or skilled nursing settings.
• Ability to comply with facility policies, including vaccine and .
• Excellent communication and interpersonal skills.
• Ability to demonstrate flexibility and teamwork in a fast-paced environment.
• Must bring a valid state or federal  wear a photo badge at all times.
