LVN​/LPN – LTC​/SNF

Job Summary:

Seeking a qualified LVN/LPN to provide patient care in a long-term care or skilled nursing facility setting. The ideal candidate will demonstrate compassion and professionalism while assisting with daily patient needs.

Responsibilities:

• Administer medications and treatments accurately following established protocols.
• Monitor and record patient vital signs and conditions regularly.
• Assist with patient hygiene, mobility, and comfort measures.
• Collaborate with the healthcare team to develop and implement care plans.
• Document patient care activities thoroughly and promptly.
• Ensure compliance with all safety and facility policies, including badge wearing and identification requirements.

Qualifications:

• Valid and active LVN or LPN license in the state.
• Minimum of 1 year of experience in LTC or SNF environments.
• Ability to prioritize multiple patient care tasks efficiently.
• Strong interpersonal and communication skills.
• Demonstrate understanding of infection control and safety procedures.
• Must have completed COVID-19 vaccination or hold an approved exemption.
