Hospice Aide, Certified Home Health Aide - Field

Northwell Health - Up to $37660.00/year -  - Responsibilities:
Provide patient care in accordance with the plan of care established by Hospice Care Registered Nurse and other interdisciplinary group members.;
Assist patients with activities of daily living, including bathing, grooming, oral hygiene, skin care, perineal care, turning and positioning, ROM exercises, ambulation, and changing bed linens.;
Maintain accurate, up-to-date documentation of patient care and ADLs; ensure records are accurate, complete, and legible.;
Perform light housekeeping duties essential to the patients health care and manage supplies, linen, and equipment; maintain a clean patient environment.;
Participate in on-call rotation, meet productivity standards, and attend in-service training as required.
