Great opportunity for Registered Nurses (RNs) who are dedicated to providing exceptional care in our Central Nurse Triage center based in Westchester, IL.
Employment Type: Full time
Shift: 7am–5pm weekdays with rotating Saturdays 8am–12pm
SummaryThe Triage Nurse will support Primary Care providers for multiple care sites in the Chicagoland area. You will play a critical role in patient access and safety by assessing symptoms, prioritizing care needs, and guiding patients to appropriate services. This role involves telephone triage, clinical decision-making, and coordination with providers to ensure timely and effective care.
Hours: 7am–5pm weekdays, rotating Saturdays 8am–12pm (may vary)

- Be the first point of care by providing compassionate telephone and online triage, using evidence-based protocols and your clinical expertise to guide patients to the right next step.
- Listen carefully, assess symptoms, and determine the best level of care—whether it’s a same-day visit, routine appointment, urgent care, or emergency services.
- Empower patients through clear education on symptom management, chronic condition support, and medication instructions, helping them feel confident and cared for.
- Work hand-in-hand with providers to coordinate timely appointments, referrals, and follow-up care that keeps patients on track.
- Keep our records accurate and complete by documenting every interaction in the EMR, ensuring seamless continuity of care.
- A Diploma from an accredited school/college of nursing (BSN or ADN)
- BLS provider (accredited by the American Heart Association) required prior to hire
- Current Registered Nurse License State of Illinois
- 5 years minimum of nursing experience in a clinical setting, preferably in a primary care or ambulatory setting.
- Experience in chronic disease management (e.g., diabetes, hypertension, asthma).
- Familiarity with population health initiatives and care coordination.
- Ability to work independently and manage multiple priorities in a fast-paced environment.
Salary Range: $29.78 - $48.84 per hour.
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
