The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises nursing assistants to ensure high-quality care in compliance with regulations. Responsibilities include planning and overseeing daily nursing activities, administering medications, completing documentation, and maintaining confidentiality. The LPN performs services such as catheterization, wound care, IV care, and fosters a collaborative environment. The role involves coordinating with physicians, ensuring safety protocols, and communicating effectively with residents, families, and staff.

• Perform all functions under your active license related to your duties.
• Know the residents you serve, providing care based on care plans and physician orders.
• Prepare and administer medications as ordered.
• Develop and maintain good rapport with staff and create a warm, calm environment.
• Assist in residents' daily activities and social engagement.
• Communicate changes in residents' needs to clinical leadership.
• Care for residents with dementia appropriately.
• Take on other duties within your knowledge and experience.

• Current unencumbered LPN license.
• Knowledge of nursing practices, laws, and regulations.
• Experience in Long Term Care preferred.
• Willingness to undertake additional training if required.
• CPR and First Aid certifications.
• Flexible, organized, and able to handle multiple priorities.
• Effective communication skills.
• Good judgment and problem-solving skills.

• Sit, stand, bend, walk; lift/move up to 100 lbs.
• Handle mental and emotional stress.
• Meet health requirements, including medical and physical exams.
• Assist in emergency evacuations.
• Sufficient visual and auditory abilities for communication.
• Moderate noise level environment.
