Job Summary

To work within the Transition Support Services (TSS), which supports residents’ recovery from addiction and moderate to severe mental health conditions and prepares for reintegration to the community or to transition to their next level of care.
To be part of a Multidisciplinary Treatment Team that will be available to support the medical and pharmacological needs of the residents in the program and deliver services under the agency clinic license and in keeping with their supervisory requirements. The RN/LPN is one who actively promotes the agency’s philosophy and maintains a high level of commitment to the program and to the health and wellbeing of the residents.

Duties

and Responsibilities
  • Conduct medical screenings on all new residents to the program within the time parameters of the program’s intake process and procedures.
  • Deliver medication management (individual/group) as required by program or resident including assistance with prescription refills, medication changes, prior authorizations, pharmacy issues, and medication reconciliation prior to a pending a hospital discharge.
  • Support medication adherence by residents and monitor for symptoms and side effects.
  • Develop strong and supportive relationships with residents and reinforce progress in each individual’s recovery process.
  • Collect health vital signs (i.e., temperature, pulse, blood pressure), documentation and reporting.
  • Maintain updated progress notes of clinical and medical interventions.
  • Maintain residents’ records as per program/clinic licensing standards and conduct medication audits to ensure compliance with regulations.
  • Provide education to residents on topics regarding health, wellness, medication, symptoms and side effects.
  • Provide staff training and education regarding health, medications and medical protocols including the use of naloxone in the event of an overdose.
  • Collaborate with doctors’ offices in providing basic medical treatment, triaging, and identifying when a higher level of medical intervention is necessary.
  • Collaborate with other care providers to coordinate medical, medication, MAT, and psychiatric needs.
  • Conduct regular inventory of medical supplies and submit acquisition orders to the Program Director in a timely manner.
  • Maintain clinic area in a sanitary, organized, orderly, safe manner.
  • Participate as a member of a multi-disciplinary team and attend one team meeting per month in which program operations and quality assurance are principal on the agenda.
  • Participate in all trainings required by the agency and additional trainings that may enhance the health, wellbeing and safety of the residents in the program.
  • Apprise Program Director of any issues or concerns regarding the operations of the clinic in a timely manner.
Minimum Qualifications
  • Valid MA LPN License
  • 1-3 Years' Experience preferred
  • Bilingual Preferred (Spanish/English)
  • 2 years experience working with psychiatric population
  • Driver's license required
