Substance Use Counselor

Health Care Resource Centers, part of HCR Home Care, seeks a Substance Use Counselor to deliver compassionate, evidence based counseling to adults in home and community settings. In this role, you will complete substance use assessments, develop individualized treatment and recovery plans, and provide individual and group counseling focused on harm reduction and relapse prevention. You will collaborate closely with an interdisciplinary healthcare team, maintain accurate EHR documentation, and ensure ethical, person centered care.

Responsibilities

• Conduct comprehensive substance use assessments for home and community based clients
• Develop and update individualized treatment and recovery plans in collaboration with patients and families
• Provide individual and group counseling using evidence based practices
• Monitor progress, adjust interventions, and document all services in the EHR accurately and timely
• Coordinate care with nurses, physicians, and community partners to support continuity of care
• Educate patients and families on substance use, harm reduction, and relapse prevention strategies
• Identify crisis situations and provide appropriate intervention or referral
• Maintain strict compliance with ethical standards, HIPAA, and organizational policies
• Participate in case conferences, supervision, and ongoing professional development
• Contribute to a culture of respect, integrity, and patient centered, home based care
