BAYADA Home Health Care is seeking a Registered Nurse (RN) for Home Health Admission in Westfield, New Jersey.

• Complete 1–3 Start of Care (SOC) visits each day in Monmouth, Middlesex, Somerset, and Union Counties.
• Conduct comprehensive patient assessments and accurately complete OASIS documentation.
• Develop individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education.
• Educate patients and caregivers on disease management, medications, and home safety.
• Monitor patient conditions and communicate changes appropriately.
• Ensure documentation is accurate, timely, and compliant with regulatory standards.

• Current Registered Nurse (RN) license in the State of New Jersey.
• Minimum one year of nursing experience; home health experience preferred.
• Experience completing OASIS documentation and Start of Care visits strongly preferred.
• Comfortable using electronic medical records; experience with Homecare Homebase (HCHB) is a plus.
• Excellent assessment, communication, and time‑management skills.
• Passion for providing exceptional, patient‑centered home health care.

• Base pay: $65.00–$75.00 per point ($101,400–$117,000 annual earning potential).
• Sign‑on bonus: $20,000 for full‑time RNs in Monmouth County.
