We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team.

As a Licensed Practical Nurse (LPN) on our evening shift, you will play a vital role in providing high-quality patient care to our residents. You will be responsible for assessing, planning, implementing, and evaluating individualized care plans to promote health, independence, and well-being. Your expertise will be essential in managing the daily care needs of our residents, collaborating with other healthcare professionals, and contributing to a positive and supportive work environment.

• Provide skilled nursing care to residents, including assessing, planning, implementing, and evaluating individualized care plans

• Administer medications and treatments as prescribed by physicians

• Perform vital signs, monitor resident conditions, and report changes to the healthcare team

• Conduct daily rounds to ensure residents are comfortable, safe, and receiving quality care

• Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to develop and implement comprehensive care plans
