Cherry Hill Nursing & Rehab is seeking an experienced and clinically strong Wound Nurse (RN Preferred) to join our skilled nursing facility in Westland, MI. This role focuses exclusively on wound care management, ensuring optimal skin integrity, regulatory compliance, and improved resident outcomes.
This opportunity is ideal for a nurse with advanced wound care knowledge and experience in a long-term care or skilled nursing setting.
Position OverviewThe Wound Nurse is responsible for the prevention, assessment, treatment, and documentation of diabetic ulcers, pressure injuries, surgical wounds, and chronic wounds. This role collaborates closely with the interdisciplinary team to promote healing, reduce facility-acquired wounds, and maintain compliance with regulatory standards.
Wound Care Responsibilities- Assess, monitor, and treat residents with pressure ulcers, diabetic ulcers, surgical wounds, and other chronic wounds
- Develop and implement individualized wound treatment plans
- Perform weekly wound rounds and maintain detailed clinical documentation
- Track and trend wound healing progress; report outcomes to the interdisciplinary team
- Collaborate with physicians, dietitians, therapy staff, and nursing team members to promote optimal healing
- Ensure compliance with CMS guidelines, infection control standards, and facility protocols
- Educate nursing staff on wound prevention strategies and evidence-based treatment techniques
- Participate in QAPI initiatives related to wound care and skin integrity
- Support survey readiness and regulatory compliance efforts
- Registered Nurse (RN) preferred;
Licensed Practical Nurse (LPN) with strong wound care experience will be considered - Active nursing license in the state of Michigan
- Wound Care Certification (WCC or equivalent) preferred
- Previous skilled nursing or long-term care experience required
- Strong knowledge of wound care standards, infection control practices, and regulatory compliance
- Excellent assessment, documentation, and communication skills
