Job Overview

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves engaging with patients in the comfort of their homes to help bridge gaps in care.

• Conduct home visits lasting approximately 45 minutes, including medication and medical history review, physical evaluation, and diagnostic tests as ordered by the health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; not required to prescribe medicine, order lab tests, or alter current treatment regimens.
• Bring a compassionate approach to every visit, building trusted relationships and connecting patients to the right care.
• Participate in the national network of purpose‑driven clinicians aiming to visit millions of people nationwide.
• Travel locally and within licensed states as needed and conduct virtual visits when required.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye examinations, etc.

Your primary workplace is the private residence of health plan members. Be prepared to work safely in environments that may contain household pets, secondhand smoke, or other allergens.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover the following primary location:
  Houghton (Counties: Gogebic, Houghton, Keweenaw, Ontonagon).

• Multi‑state licenses.

• Anticipated weekly hours: 29.5.
• Pay range: $41.85 – $90.13 per hour.
