Nurse Practitioner or Physician Assistant for our Skilled Nursing Facility (SNF)

Compensation: $100,000 - $200,000 Annual Salary

We are looking for a dedicated Nurse Practitioner or Physician Assistant for our Skilled Nursing Facility (SNF) in Westland, MI. This is a full-time, in-person position, Monday through Friday, 9 to 5, with no nights, no weekends, and no on-call. As the primary care and rounding provider in the facility, you will manage a panel of long-term care and geriatric patients, working independently from facility staff while collaborating on patient care.

Qualifications of the Nurse Practitioner or Physician Assistant:

  • Active Michigan NP or PA license in good standing
  • Experience in skilled nursing, long-term care, or geriatric primary care preferred
  • Comfortable rounding independently and managing a full patient panel
  • Strong assessment, documentation, and EMR skills

Responsibilities of the Nurse Practitioner or Physician Assistant:

  • Serve as the primary care and rounding provider for patients in the facility
  • Conduct assessments, manage chronic conditions, and adjust care plans
  • Document visits accurately and maintain compliance with facility and regulatory standards
  • Participate in patient care meetings and coordinate with the interdisciplinary team
