Position Summary

Abrazar is seeking an experienced, mission-driven CalAIM Housing Director to lead housing-focused Community Supports for Medi‑Cal members experiencing homelessness, housing instability, and complex health challenges. The Director oversees Housing Transition Navigation Services, Housing Deposits, Housing Tenancy and Sustaining Services, and Transitional Rent referrals.

This is a non-clinical leadership position for a housing or social-services professional who is ready to advance beyond a manager-level role. The Director supervises Housing Care Navigators and related staff; translates CalAIM requirements into effective workflows; monitors documentation, billing, compliance, and outcomes; and provides sound leadership in complex member situations.

The Director reports to the CalAIM Director and serves as the principal operational leader for CalAIM Housing. Regular fieldwork throughout Orange County includes partner meetings, member-support activities, housing coordination, and landlord and community engagement.

Essential Responsibilities Program Leadership and Staff Supervision
  • Direct, supervise, coach, and hold accountable Housing Care Navigators and related housing-program staff.
  • Lead team meetings, case consultations, workflow reviews, onboarding, training, and professional-development activities.
  • Set clear expectations and monitor productivity, service quality, timeliness, documentation, billing readiness, and member outcomes.
  • Manage performance through regular feedback, evaluations, improvement planning, and corrective-action processes in partnership with the CalAIM Director and Human Resources.
  • Cultivate a collaborative, trauma-informed, culturally responsive, and member-centered team environment.
  • Provide operational leadership and continuity in the absence of the CalAIM Director.
Program Operations, Compliance, and Improvement
  • Lead day-to-day implementation of CalAIM Housing Community Supports, Transitional Rent referrals, and related housing workflows.
  • Translate DHCS, managed-care-plan, and agency requirements into practical policies, procedures, tools, staff training, and service standards.
  • Oversee CES coordination, referral pathways, placement meetings, housing-match processes, and alignment with Orange County Coordinated Entry requirements.
  • Serve as the program-level HMIS lead or administrator, including user access, data quality, troubleshooting, reporting, documentation standards, and training.
  • Monitor documentation, service authorization, billing support, data integrity, confidentiality, fair-housing compliance, and audit readiness.
  • Review program data, service metrics, housing outcomes, grants, and deliverables; identify performance gaps and implement corrective solutions.
  • Maintain current knowledge of CalAIM Housing Community Supports, Medi‑Cal requirements, CES/HMIS standards, housing resources, and relevant policy changes.
Member Support and Service Oversight
  • Guide staff supporting individuals, families, and youth experiencing homelessness or housing instability, including members with complex medical, behavioral-health, or substance-use needs.
  • Review assessments, eligibility determinations, housing plans, service documentation, and Transitional Rent referrals for accuracy, appropriateness, and compliance.
  • Resolve complex, high-acuity, escalated, or time-sensitive situations involving housing access, placement, tenancy, stabilization, and retention.
  • Support housing searches, landlord engagement, move-in coordination, tenancy concerns, benefits access, and stabilization strategies.
  • Maintain a limited caseload or provide temporary member coverage when needed for continuity of operations, while keeping leadership and oversight as the position's primary focus.
