About the Mental Health Transitional Living Homes

The CDHS Office of Civil and Forensic Mental Health’s Transitional Living Homes Program was created to increase bed capacity for persons in need of residential behavioral health treatment. House Bill (HB) 22‑1303 has authorized new beds to be used for persons needing residential services at a less restrictive level of care and to reduce and eliminate the backlog of such persons in more restrictive settings.

HB 22‑1303 directs the Department of Human Services and the Department of Health Care and Policy Financing to create, develop, or contract to add at least 125 additional beds at mental health residential treatment facilities throughout the state for adults in need of ongoing supportive services. The bill also requires that the department renovate a building at the mental health hospital at Fort Logan to create at least 16 additional civil beds for such persons.

These beds serve as a step‑down option from inpatient care and as a step‑up when deemed medically necessary and lower‑level of care settings can no longer adequately and safely provide effective care.

This position provides direct hands‑on milieu management care to clients. Duties include assistance with activities of daily living, documentation of care, awareness of psychosocial and physical needs and safety, engagement in social and recreational activities, and support for continued development of social and life skills. Services are performed consistent with professional, legal, and regulatory standards. The role is full‑time, in‑person, essential services designation, and requires 24/7 care and treatment.

Shifts include daytime, evening, and overnight, including weekends.

Responsibilities Residential Client Care and Safety/ Security
  • Provide direct client care, including hands‑on performance and assistance with activities of daily living and continued development of social and life skills through recreational and social activities.
  • Arrange and adapt personal care items and physical environment needs to support ongoing client needs.
  • Guide and instruct clients for self‑care activities as part of their continued development of activities of daily living.
  • Assist clients in becoming oriented to the program, reviewing requirements, responsibilities, and inventory of personal items.
  • Supervise clients and ensure their needs are being met on a daily basis.
  • Monitor safety needs, supervise whereabouts and interactions, and refer to the MHTL Homes team regarding potential changes.
Transportation
  • Transport clients to and from appointments and recreational or social activities when necessary.
  • Ensure safety of clients while transporting, adhering to safety laws such as seat‑belts and road rules.
  • Maintain a schedule of outside appointments and activities, collaborating with the MHTL Homes team.
  • Complete trip logs in a timely manner.
Department Standards and Staff Development
  • Adhere to department standards and staff development protocols.
  • Ensure that MHTL Homes programming and operation standards are followed per policies and regulatory requirements.
  • Complete required staff training, including MHTL Homes specific training and CDHS mandatory training.
  • Attend assigned meetings and in‑services as scheduled.
  • Complete all documentation (shift notes, incident logs, refrigerator temperature logs, and other logs related to life, safety, and health) promptly and accurately.
  • Resource and orient new or pulled staff on milieu management and safety basics.
Medication Administration
  • Administer medications in accordance with the Medication Administration Record (MAR).
  • Administer medications at appropriate doses and times.
  • Record medication details, client responses, refusals, and relevant mental status information.
  • Be certified as a Qualified Medication Administration Professional (QMAP); the program assists un‑certified staff in obtaining certification.
