Registered Nurse - RN Case Manager

About the Role

Make a difference in the lives of former nuclear weapons workers by providing in‑home nursing and case management services. You will work on a flexible schedule and earn $44 per hour, including full pay for drive time.

• Conduct weekly in‑home nursing visits (1–2 hours) for clients with chronic or terminal illnesses.
• Perform physical assessments, including vitals, and ensure home safety with medical equipment and supplies.
• Fill weekly medication planners and oversee skilled and non‑skilled caregivers.
• Provide direct care and case management for assigned clients.
• Collaborate with the regional director and other personnel to identify and correct issues and improve services.
• Plan, implement, and evaluate care; coordinate client care conferences.
• Serve as local on‑site clinical resource and support staff scheduling and competencies.
• Adhere to agency policies, nursing delegation guidelines, and scope of practice.
• Perform supervisory visits, review documentation, and contribute to performance improvement initiatives.
• Assist with staffing, training, education, and competency validation.
• Document and submit data on infections, occurrences, complaints, and grievances.
• Assist with marketing activities when requested.

• Graduate of a state‑approved school of professional registered nursing (BSN preferred).
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of 2 years of nursing experience, including at least 1 year in home care or a closely related field.
• Minimum of 1 year of supervisory or case‑management experience (preferred).
• Current CPR certification.
