Compensation (Pay): $32.39–$46.96 hourly, based on experience. An additional $2.00 per hour if you hold a BSN.

Sign‑On Bonus: $5,000 paid in four installments: $1,250 after 30 days, $1,250 after 90 days, $1,250 after 6 months, and $1,250 after 9 months. Current employees are not eligible.
Loyalty bonus: $5,000 after 1 year of service and $1,250 for every additional 3 months (full‑time only, prorated for part‑time).

Position Location: Pediatric Walk‑in Clinic – 2828 International Circle, Suite 160, Colorado Springs, CO 80910

Hours: Monday through Saturday, 8 am to 8:30 pm. Rotating 12‑hour shift, 36 hours per week.

Job Summary: The Care Team Nurse functions as a liaison between the patient and members of the Care Team to optimize health status and quality of life, performing routine and emergent nursing functions in alignment with the Colorado Nurse Practice Act.

Essential Duties and Responsibilities
  • Assesses and triages based on comprehensive risk assessment.
  • Evaluates patient readiness for change, physical and psychosocial needs.
  • Links patients to resources, social support, and family.
  • Assesses high‑risk patients per Care Management policies.
  • Facilitates access to cost‑effective care.
  • Collaborates with community partners.
  • Provides education to patients, families, and caregivers.
  • Encourages patient engagement and self‑management.
  • Provides outreach to individuals and the community.
  • Conducts in‑reach for inactive patients.
  • Utilizes technology such as Electronic Health Records, state registries, and Health Information Exchange.
  • Documents all interactions accurately; follows charting and billing protocols.
  • Maintains healthy relationships with team members and staff.
  • Supports clinical training; ensures Medical Assistants are competent.
  • Assists providers with inbox tasks, including medication refills and paperwork.
  • Delegates follow‑up needs for labs and outside testing.
  • Advocates for patients.
  • Provides education on disease processes and medication regimens.
  • Works with care teams to review schedules and prepare charts.
  • Understands immunization schedules and supports clinic staff.
  • Operates corporate or personal vehicles safely; travels to Peak Vista facilities as needed.
  • Promotes Peak Vista’s mission, vision, and values.
  • Develops relationships with all departments and leadership.
  • Embraces cultural diversity.
  • Manages human, financial, and material resources; safeguards data.
  • Strives for best outcomes and quality service.
  • Adheres to the Compliance Plan and conflict‑of‑interest requirements.
  • Contributes to performance improvement goals.
  • Other duties as assigned.

Qualifications: Ability to perform all essential duties; reasonable accommodations available. Spanish bilingual preferred.

Education and Experience
  • Graduate of an accredited nursing program; BSN preferred.
  • Two years’ experience in case management and clinical nursing preferred.

Skills:

Basic; ability to use intra/internet, manage timecards, review policies, use email, enter and correct data, format worksheets, print, create simple PowerPoint presentations.

Certificates and Licenses
  • Current Colorado RN license and current CPR (BCLS) certification.
  • Minimum age 21 to be an eligible driver. Valid Colorado driver’s license and proof of insurance required.
  • Must meet and maintain driver criteria as prescribed by Peak Vista’s insurance underwriter.
