Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. You’ll engage with people in the comfort of their own homes and help bridge gaps in care by building trusted relationships and delivering comprehensive health evaluations at the front door.
Responsibilities- Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes.
- Visit millions of people nationwide to connect them with the right care for their unique needs.
- Work with Signify Health as a clinician colleague and fulfill the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed, and conduct virtual visits when requested.
- Provide Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams, bone density screening).
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Multi‑state licenses (preferred).
- Ability to cover primary locations:
Fremont, Saguache, Custer, Alamosa, Costilla, Huerfano, Pueblo, Crowley, Otero, Las Animas.

The typical pay range for this role is: $41.85–$90.13 per hour or equivalent annual salary.
