In-Home Health - Nurse Practitioner or Physician Assistant; Per Diem - Grand Junction, CO - Mesa

Overview

Signify Health, part of CVS Health®, is seeking a Per‑Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves visiting people in their own homes, conducting a 45‑minute visit that includes medication and medical history review, a physical evaluation, and any diagnostic tests ordered by the individual’s health plan. Clinicians use an iPad and a streamlined clinical workflow and are not required to prescribe medication or alter current treatment regimens.

• Conduct in‑home health evaluations, including medication and medical history review, physical assessment, and diagnostic testing such as diabetic eye exams and spirometry.
• Build trust and engage patients one‑on‑one to address health questions and concerns.
• Travel locally and, as required, within licensed states to reach patients.
• Perform virtual visits as needed.
• Provide diagnostic and preventive services specified by Signify Health, including spirometry and diabetic eye exams.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to work in a variety of home environments, including exposure to pets, smoke, or allergens.
• Strong communication and interpersonal skills.
• Valid driver’s license and reliable transportation for travel.

Primary coverage area:
Grand Junction /Mesa County.

Anticipated weekly hours: 12 (part‑time, per‑diem).

Hourly base rate range: $41.85 – $90.13. Pay varies with experience, education, geography, and other factors.

• Hourly base rate plus additional incentives (e.g., per visit completed).
• Malpractice insurance coverage during Signify Health‑related activities.
