In-Home Health - Nurse Practitioner or Physician Assistant Colorado; Traveler

Overview

Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations across a defined coverage area. The role requires traveling up to 100% of the time, obtaining multiple state licenses as needed, and performing evaluations in patients’ homes.

• The visit lasts about 45 minutes, including a medication and medical history review, a physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the person’s health plan.
• The clinician uses an iPad and an intuitive clinical workflow for each evaluation and is not required to prescribe medication, order lab tests, or alter the current treatment regimen.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Receive additional per‑completion incentive pay for all in‑home visits.
• Be flexible to travel locally and within licensed states, up to 100% of the time.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi‑state licenses in coverage area, as required.
• Able to cover all counties within Colorado and flex to other areas of business need.
• Hold multiple state licenses will be required for this role.
• Eligible to travel up to 100% of the time and work in diverse home environments.

• Anticipated weekly hours: 40.
• Time type: full time.
• Pay range: $87,035.00 – $ (base hourly rate or base annual full‑time salary).
