In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Denver, CO - Arapahoe

About the Role

Signify Health, part of CVS Health®, is seeking a part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role involves conducting 45‑minute visits that include medication and medical history review, a physical evaluation, and, when ordered by a health plan, additional diagnostic tests such as diabetic eye exams or spirometry. The clinician uses an iPad and an intuitive workflow and is not required to prescribe medication, order lab tests, or alter current treatment plans.

• Work with Signify Health as a clinician colleague, fulfilling the responsibilities of the specified role.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry and DEE.

The primary workplace is the private residence of health plan members. Clinicians must be able to perform duties safely in homes that may include household pets, secondhand smoke, or other airborne allergens or irritants.

• Active, unrestricted nurse practitioner or physician assistant license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover the following primary locations:
  Denver / Arapahoe County.

Pay range: $41.85 – $90.13 per hour. Anticipated weekly hours: 29.5. Part‑time employees qualify for selected benefits, including malpractice insurance coverage during Signify Health‑related activities and supplies and other perks. Additional benefit details are available in the benefits guide.
