Hospice & Palliative Care Part‑Time Physician

For more than 35 years, Bridging Life (formerly Carroll Hospice) has delivered nationally recognized compassionate care to patients and their families. We have expanded our services across the Life Bridge Health continuum of care to serve people living with serious illness in palliative care, home hospice care, inpatient hospice care, and grief support.

• Attend and actively participate in weekly Interdisciplinary Group (IDG) meetings, providing clinical leadership guidance and medical expertise.
• Certify and recertify patient eligibility for hospice care, ensuring clinical findings support eligibility.
• Complete required narratives for certificates of terminal illness.
• Participate in development, implementation, and revision of initial and subsequent comprehensive plans of care (POC), including interventions to manage pain and symptoms.
• Provide accurate and timely documentation of all patient care services personally provided.
• Assume responsibility for medical care of patients receiving hospice care in conjunction with the attending physician and IDG, including prescribing medications and signing death certificates.
• Review medication regimens for effectiveness, side effects, interactions, and duplication, and assist the IDG in measuring outcomes.
• Participate in an equitable share of on‑call coverage (overnight, weekend, holiday) and bedside rounding at inpatient facilities.
• Advising management and staff on complex care issues and resolve conflicting patient‑care decisions with the attending physician.
• Optimize the comfort and dignity of each patient, providing care consistent with patient and family needs and goals.
• Assist the Medical Director in managing inpatient and outpatient palliative care programs; clinical or administrative work may be an option.
• Serve as physician designee in the absence of the Medical Director.
• Other reasonable duties assigned by the Medical Director or a designee.

• Board‑certified in Hospice & Palliative Medicine or equivalent advanced experience in hospice and palliative care medicine.

• Ability to work part‑time (20 hours per week / 0.5 FTE).
