Case Manager Rn Wknd

Job Summary

The primary emphasis of the RN Case Manager is to partner with all other members of the health care team to assure efficient care of inpatients, avoiding delays and denials whenever possible. The Case Manager, working with the health care team, strives to achieve the best outcomes of appropriate utilization and safe, efficient and timely transition and discharge planning functions. Required to work weekends.

• Required Associate's Degree
• 2-4 years of related experience and/or training or an equivalent combination of educational experience; UM/CM/Discharge Planning experience preferred and required
• CCM – Certified Case Manager
• CPR – AHA Healthcare Provider
• RN – Registered Nurse

Required to work weekends.

Experience with complex inpatient settings, advanced patient assessment, and interdisciplinary collaboration.
