Pride Health is hiring Home Health RNs to support our client in Westminster, MD 21157
. This is a 6+Months assignment (Possibility of extension). opportunity and a great way to start working with a top-tier healthcare organization!
This role focuses on delivering high-quality in-home nursing care, care coordination, OASIS documentation, and post-acute patient management.
Essential Duties and Responsibilities- Initiate, develop, implement, and revise individualized patient care plans in collaboration with physicians and interdisciplinary healthcare teams
- Provide skilled nursing care in patient homes according to physician orders and agency protocols
- Complete comprehensive patient assessments including OASIS documentation and medication reconciliation
- Supervise and coordinate care provided by Home Health Aides and LPN/LVNs
- Conduct required supervisory visits in compliance with federal and state regulations
- Maintain accurate and timely clinical documentation according to payer and agency requirements
- Ensure infection prevention and patient safety standards are consistently followed
- Coordinate care transitions and monitor patient progress throughout the episode of care
- Sync documentation and visit notes within required timelines
- Active Massachusetts RN License required
- Strong OASIS documentation experience preferred
- Ability to work independently and manage patient schedules autonomously
- OASIS Experience - Required
- 1-2 years' experience as a Registered Nurse in a clinical care setting
- 2+ years' Home health experience.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelors of Science in Nursing

Local Pay range$50-$58/hour
