Responsibilities:
- Provide direct patient care, including vital sign checks, baths, grooming, and dressing
- Assist with daily living activities, such as feeding, toileting, and ambulation
- Monitor and report patient condition to nursing staff
- Maintain accurate and timely records of patient care
- Communicate with patients, families, and healthcare team members
- Participate in quality improvement initiatives to improve patient outcomes
