What You Will Do

Are you ready for the rewards and challenges of an outreach role? As an important contributor to the Department of Citizen Services, you will prepare and deliver trainings in support of person-centered care within assisted living facilities and nursing homes. Under the direction of the Program Manager, you will support efforts to foster environments that enhance resident choice, dignity, autonomy, and quality of life.

The work you do will support compliance with all applicable laws and standards.

Essential Duties

• Prepare and deliver trainings on elder abuse prevention, resident rights, and person-centered care to local care facilities residents, family members, staff, as well as to Fire/EMS staff, community partners, and the general public.

• Create educational materials on the Long-Term Care Ombudsman Program for distribution.

• Perform frequent visits to local care facilities. With a trauma informed approach, develop rapport with residents, family members, and facility staff.

• Assist in the establishment of Culture Change Committees, collaborative committees promoting person-centered care within nursing homes.

• Support efforts in nursing homes to foster environments that enhance resident choice, dignity, autonomy, and quality of life.

• Develop and maintain effective working relationships with nursing home and facility staff, residents, and families.

• Assist with complaint investigation management. Determine appropriate course of action. Conduct field visits as required.

• Establish, organize, and maintain files and electronic records management/file systems.

• Ensure all work is complete and timely to support department goals. Evaluate feedback and procedures to improve efficiency.

• Invite collaboration and seek learning opportunities. Proactively assist peers to create a supportive work environment.

You Might Be a Good Fit If

Education and Experience

• Associate degree or two years of college in a related field

• Two or more years of related experience

• A comparable amount of education and experience may be substituted for the minimum requirement.

Required Licenses and Certifications

• Valid driver's license

• Ombudsman certification (or must obtain within 6 months of employment)

Additional Information

• Requires criminal background check as condition of employment.

• Must meet Carroll County Risk Management driver eligibility requirements

Working Conditions

• Occasional exposure to potentially adverse environmental conditions, including, but not limited to, chemicals, noise, and airborne particles.

• Must wear appropriate PPE as required.
