What You’ll Do
  • Conduct in-home supervisory visits to monitor care, safety, and performance
  • Provide real-time coaching and mentoring in ADLs/IADLs, dementia care, fall prevention, and infection control
  • Support caregivers with retraining, recognition, and advocacy
  • Document supervisory visits in our CRM/EVV system same day
  • Ensure compliance with EVV, HIPAA, and professional standards
  • Collaborate with the Nurse Director and Scheduling Team for caregiver-client matches
  • Escalate safety concerns and participate in Root Cause Analysis (RCA) follow-ups
  • Rotate flexible schedules (day/evening visits, every other weekend)
What We’re Looking For
  • CNA, LPN, or RN with supervisory or leadership experience
  • 5+ years caregiving or clinical experience (home care, assisted living, rehab, or long-term care)
  • Strong knowledge of dementia care and chronic condition support
  • Excellent communication and leadership skills
  • Tech-savvy with mobile documentation (CRM/EVV)
  • Valid driver’s license + reliable transportation (company vehicle may be available)
  • CPR preferred
