Position: Community Care Nurse, HCT (RN)

The Community Care team is a multidisciplinary group including Registered Nurses (RN), Licensed Practical Nurses (LPN), Community Social Workers (CSW), and Community Health Coordinators (CHC). They work with high-need patients and their primary care physicians to address medical and social needs, engaging patients fully in our primary care model to maximize their time at home.

The RN Community Care Nurse will serve as a clinical lead, coordinating efforts to stabilize high-risk patients, focusing on safe transitions from facilities, stabilization of ambulatory patients, and outreach to unengaged patients. Responsibilities include initial assessments, care plan development, clinical supervision, and team prioritization. The role may also include direct supervision of some team members and adherence to departmental standards and policies.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

  • Provide in-home and telephonic visits to high-risk patients to prevent hospital admissions and readmissions.
  • Perform initial assessments and develop care plans for follow-up by LPNs; review patient charts and conduct discharge procedures.
  • Conduct supervisory visits to educate patients and oversee proper discharge from case management.
  • Perform clinical and Social Determination of Health (SDoH) assessments, including disease monitoring, medication management, and health education.
  • Coordinate the Plan of Care:

  • Oversee LPNs with clear care plans and education during visits.
  • Conduct initial assessments to determine outpatient needs.
  • Ensure care plans reflect patient needs and available community services.
  • Collaborate with patients, families, and care team members to complete care plans.
  • Communicate instructions effectively to ensure proper implementation.
  • Assess safety, caregiver capacity, educational needs, and environment.
  • Coordinate multidisciplinary team meetings and follow-up.
  • Assist patients with navigating healthcare systems and accessing community resources.
  • Maintain communication with families and providers to promote patient well-being.
  • Build supportive relationships to encourage self-management.
  • Monitor outpatient service quality and appropriateness.
  • Facilitate access to community and financial resources, referring to social workers as needed.
  • Perform urgent home visits under physician direction.
  • Perform other duties as assigned.
  • KNOWLEDGE, SKILLS AND ABILITIES:
    • Excellent interpersonal and communication skills in diverse communities.
    • Critical thinking and autonomous work ability.
    • Experience in monitoring patient progress and adjusting care plans.
    • Knowledge of nursing practices, patient documentation, and community health resources.
    • Strong organizational and coordination skills.
    • Effective communication of technical information.
    • Proficiency in Microsoft Office and other software.
    • Willingness to travel up to 10% locally, regionally, and nationwide.
    • Fluency in English; bilingual skills a plus.
    • Independent judgment and decision-making skills.

    EDUCATION AND EXPERIENCE CRITERIA:

    • Associate degree in Nursing required; BSN or related bachelor’s degree preferred.
    • Valid RN license in the state of employment required.
    • Minimum 2 years of clinical experience; 1 year in community case management preferred.
    • Case management certification preferred.
    • Valid driver’s license and BLS certification required within 90 days of employment.
