Responsibilities- Review patient referrals to assess home care needs and coordinate appropriate services.
- Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care.
- Review and approve revisions to patients' care plans efficiently through EMR workflows.
- Utilize EMR, email and other systems effectively to enhance communication with your clinical staff.
- Lead and participate in the interviewing, onboarding, and training of new hires at your center.
- Facilitate and conduct planning, implementation, and evaluation of team education needs.
- Conduct morning briefings on-site once a week and remotely for the remaining days.
- Create and manage the nursing weekend rotation schedule.
- Lead and support a team of home health clinicians providing care in the community.
- Facilitate daily clinical meetings and coordinate patient care and workflow.
- Oversee admissions, visit scheduling, care planning, and physician orders.
- Support staff with clinical decision-making and complex patient situations.
- Ensure timely and accurate clinical documentation and regulatory compliance.
- Participate in hiring, onboarding, coaching, and performance management of team members.
- Collaborate with physicians, community partners, and internal teams to coordinate care.
- Active license as an RN, PT or OT in MA.
- Maintain current BLS CPR certification.
- Minimum of two (2) years of direct clinical experience.
- Prior supervisory experience preferred.
- Excellent written and verbal communication skills.
- Proficient with computers, tablets, smartphones, EMR systems, email, and other electronic systems.
- Ability to foster a positive and purposeful culture.
- Salary Range: $100,000 - $120,000
