Pathways Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team. This role provides direct patient care in a home health setting while collaborating closely with RNs, physicians, patients, and families to ensure high-quality care and positive patient outcomes.
Service Area:- Westwood, Quincy, Brockton, Walpole, Marshfield, Weymouth, Holbrook, Randolph areas.
- Provide skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care.
- Monitor and document patient condition, vital signs, symptoms, and responses to treatment.
- Administer medications and treatments as prescribed.
- Assist with implementation and ongoing evaluation of patient care plans.
- Educate patients and caregivers on medications, treatments, and disease management.
- Communicate changes in patient condition to the supervising RN and physician as appropriate.
- Maintain accurate and timely clinical documentation.
- Promote a safe and comfortable environment for patients.
- Coordinate necessary supplies, equipment, and services for patient care.
- Support quality improvement initiatives and compliance standards.
- Accept and fulfill assigned patient visits and schedules reliably.
- Current and active LPN license in Massachusetts.
- Graduate of an accredited practical nursing program.
- Previous nursing experience preferred, especially in home health, hospice, or geriatrics.
- Proficient with electronic medical records (EMR).
- Excellent communication and interpersonal skills.
- Current BLS CPR certification.
- Valid driver’s license and reliable insured vehicle required.
