Join a growing home health organization as it builds out its Medicare home health division. This is a high-impact RN opportunity blending field-based patient care with operational influence as the program scales.
Key Details- Location: Newton/Westwood + South Shore territory
- Schedule: Monday-Friday, flexible between 7:00 AM – 7:00 PM (based on patient needs)
- Hours: 40 hours/week
- Travel: ~60% field-based travel
- Pay Range: $48-52/hr
This role is ideal for an RN who wants more than a standard visiting nurse position. You’ll be part of launching and shaping a Medicare-focused home health program, helping build workflows, supporting compliance, and influencing how care is delivered as the division grows. It’s a strong fit for nurses who want autonomy, flexibility, and a chance to be part of something expanding.
Responsibilities- Conduct patient evaluations in homes and rehab facilities
- Complete OASIS assessments (SOC, ROC, Discharge)
- Develop, implement, and coordinate patient care plans
- Determine appropriate visit frequency and needed services
- Maintain accurate, Medicare-compliant documentation
- Collaborate with intake, billing, clinicians, and caregivers to coordinate services
- Travel throughout the assigned territory to support patient care
- Active RN license in good standing within Massachusetts
- Prior OASIS experience or be able to complete OASIS training (see below)
- 1+ year of home health or community nursing experience
- Familiarity with Medicare regulations
- Strong clinical assessment and critical-thinking skills
- Reliable transportation and willingness to travel throughout the territory
